This form is for students in district sponsored team sports, marching band, & JROTC.

• PE Credit for Sports

This form is for PE credit not sponsored by the school district.

• Community Service Approval and Verification Form

You can receive .5 credit for every 120 hrs of Community Service. Have a supervisor fill out and sign and return to counseling department.

• Work Credit Form

You can receive .5 credit for every 120 hrs you work.

• Youth Court Service Form
